Job overview
An exciting opportunity has arisen for a Nurse Practitioner to work as a permanent integral member of our DVT Service.
Our Deep Vein Thrombosis (DVT) / Anticoagulation Service is a community-based, nurse-led, DVT assessment and treatment service for mobile and immobile patients.
Our aim is to reduce the number of hospital admissions for immobile people with DVT and to reduce the amount of time that patients with pulmonary embolisms or on anticoagulation therapy have to stay in hospital. We also provide education and advice to other healthcare professionals.
Main duties of the job
* To support the Deep Vein Thrombosis (DVT) team in the delivery of high quality care based on patients' individual needs.
* Assist in the provision of a nurse led screening, investigation and treatment service for patients with suspected and/or proven DVT.
* To provide high quality care in a number of settings e.g. patient's home, clinic etc.
* To provide a comprehensive DVT follow up service for treatment and monitoring of patients' condition.
* To prevent unnecessary hospital admission and/or prolonged hospital stay.
* To assist in the initiation and monitoring of anticoagulation therapy.
* To administer medications using patient group directions and/or non-medical prescribing.
Person specification
Qualifications and professional training
* Current Registration with NMC First level nurse RGN
* Educated to degree level or equivalent
* Evidence of relevant recent professional development
* Intermediate Life support training (ILS)
* Clinical Examination skills at minimum of degree level
* Clinical Examination skills at masters level
* Clinical diagnostic skills at minimum of degree level
Experience and Knowledge
* Minimum of 5 years post registration experience in acute community setting, undertaking nurse practitioner role
* Knowledge of Deep Vein Thrombosis and contributing factors, screening and treatments.
* Knowledge of the current Wirral anticoagulation guidelines.
* Knowledge of the current Wirral clinical protocol for the use of low molecular weight heparins.
* Knowledge of and experience in using PGDs
* Knowledge and understanding of the purpose and content of relevant clinical guidelines
* Experience of exploring innovative ways of working and ensuring best practice within the clinical area
* Experience of autonomous clinical decision making
* Understanding of clinical governance and risk assessment
Skills and attributes
* Able to work as part of a team and have the ability to make decisions and work autonomously when necessary.
* IT literate
* Possess a current driving licence and have access to a car for work purposes.
* Knowledge of professional and NHS Issues
* Ability to work efficiently and autonomously
