The Community Mental Health HUB is looking to recruit a full-time Band 6 Mental Health Practitioner to support the duty system.
The HUB duty system is available for all service users accepted to the HUB for support in the absence of their keyworker, along with liaising with families, carers, and other professional agencies.
This is a full-time post consisting of 37.5 hours per week, 9am to 5pm Monday - Friday, but we also have a part-time post available at 22.5 hours per week.
You must be innovative, have good communication skills, be approachable, and committed to providing a quality service to our service users and their carers/family.
We are looking for someone who wants to grow and develop their knowledge and skills by working closely with colleagues, sharing experience and knowledge, and being a reflective practitioner who continues to learn from the team and each other.
You will be part of a multi-disciplinary team working with other community-based services, supporting service users with a range of mental health needs at varying stages of recovery.
As a valued member of the team, you will have an opportunity to shadow in other areas of the Mental Health HUB and wider service, as part of the induction process.
Main duties of the job
The function of duty is to provide timely support and assistance to clients who are struggling with their mental health and experiencing emotional distress. These clients will be under the care of the Hub and may have a Keyworker or be awaiting intervention. This support may include:
1. Emotional support: Offering a compassionate and non-judgmental presence to clients, listening to their concerns, and providing reassurance.
2. Assessment: Conducting brief assessments to understand the client's current mental health status, risks, and determine appropriate interventions.
3. On-going Support: Connecting clients with additional resources, services, or support groups, for ongoing care and assistance.
4. Psychoeducation: Providing clients with information and resources to help them better understand their mental health issues and develop coping strategies.
5. Collaborative problem-solving: Supporting and encouraging clients to identify challenges and develop effective coping strategies and plans to address their needs and goals.
6. Home visits: May be required prior to referring to the Crisis and Home Treatment Team (CRHTT).
Overall, the duty provision plays a crucial role in offering support and assistance to clients in distress, helping them navigate their mental health challenges and access the appropriate resources for ongoing care and support.

Job responsibilities
1. To make contact with patients recently discharged from hospital who are not allocated a keyworker but have an open referral to the HUB (3 day follow up).
2. As a member of the multidisciplinary team, to be responsible for the assessment of need and the planning, implementation, and evaluation of treatment plans incorporating a network of community services and resources.
3. To offer both individual and specialist group treatments and to deliver direct care, according to individual treatment plans and in response to identified need.
4. To monitor and evaluate the effectiveness of all interventions at regular intervals.
5. To offer active involvement to service-users, their families, and other significant persons in care planning and management.
6. To contribute to the maintenance of positive contacts between the service and the families/carers of service users and to encourage their active participation in plans offering carers assessment and intervention as needed.
7. To have an awareness and recognition of the impact of race and culture in mental healthcare provision and to ensure the service users' views are taken into account as far as possible and enable the service users to establish their own goals when planning individual programmes.
8. To maintain up-to-date written and electronic records of all interventions, to record information and write reports as necessary. To use the integrated mental health services documentation in line with local policies.
9. To take a lead in team meetings, to participate in clinical meetings and to lead where necessary handover of care to all staff involved in care. This should include support and guidance to junior staff, ensuring that the nursing contribution is professional and systematically applied.
10. To ensure other parts of the mental health services are promptly informed of known service users accessing the Team. To maintain a good network of communications throughout the services.
11. To ensure that risk is systematically assessed and managed in accordance with Trust Policy.
12. To ensure, through good clinical practices, that all aspects of the Mental Health Act 2007 and the NHS and Community Care Act 1990 are being adhered to.
13. To ensure the requirements of all current legislation are observed.
14. To participate in medication management Rota if required.
15. To participate in the delivery of Group interventions.

Qualifications
Essential
1. Professional Qualification at Diploma or Degree level in mental health nursing, social work, or occupational therapy.
2. Student supervision/mentoring qualification.
Experience

1. Demonstrable post-basic experience in Mental Health.
2. Sound knowledge base.

1. Experience of working in the community.
