A fantastic opportunity has arisen for a committed, motivated, and driven individual to work as an Exercise Instructor in the Community Pulmonary Rehabilitation service. The role will include working across community venues, in patients' homes, being a core member of the multi-disciplinary team, and empowering patients to make informed choices about their health and healthcare.
This role demands the ability to be innovative, flexible, a strong team player, and able to work autonomously. Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to transport for work purposes.
You will need to work within your own scope of practice and be able to provide pulmonary and exercise rehabilitation support to patients with a respiratory diagnosis.

Main duties of the job
To work as part of the KCRS in the delivery and co-ordination of exercise-based Pulmonary Rehabilitation ensuring high quality, timely, and evidence-based care. To provide evidenced-based care to patients with a variety of underlying chronic respiratory diseases, including but not limited to COPD, bronchiectasis, asthma, and pulmonary fibrosis.
Job responsibilities
PRINCIPAL ACCOUNTABILITIES
CLINICAL
* To assess, plan, treat, and review the exercise needs of patients referred for Pulmonary Rehabilitation with an underlying chronic respiratory disease.
* To deliver the exercise element and participate in the education program of Pulmonary Rehabilitation.
* To ensure that patients progress appropriately through sessions and give advice and information as necessary.
* To refer patients to appropriate community support programmes.
* To communicate effectively with members of the multi-disciplinary team to ensure holistic patient care.
* To provide PR in a variety of clinical settings, including leisure centres, community venues, gyms, virtual platforms, home-based, and utilizing exercise prescription platforms.
* To be able to work as part of a team, but also to work independently in a variety of clinical settings.
MANAGERIAL/LEADERSHIP
* Maintain accurate and legible records of patient care and activity in line with departmental guidelines and trust policies.
* Assist with the organisation and delivery of Pulmonary Rehabilitation.
* To support the development of students who may be on placement within the service.
* To support in the development of assistant staff within the service.

Person Specification
Qualifications & Education
Essential
* Evidence of study at degree level in Sport/Exercise Science, Exercise Physiology.
Desirable
* Postgraduate training in Pulmonary Rehabilitation.
Knowledge & Experience
Essential
* Good knowledge of chronic respiratory disease, co-morbidities, exercise assessment and prescription, exercise physiology.
* Aware of own scope of practice and appreciate the need to seek advice from clinical colleagues, as required.
Desirable
* Experience of working within a Cardiac Rehabilitation programme.
* Experience of working with groups.
* Experience of health promotion.
* Previous experience in audit or research.
