The role is for a bank band 5 medicine management technician. The hours are 9-5pm including late shifts and weekends as per rota.
Main duties of the job
The post holder will:
1. Have experience of working in a hospital pharmacy post qualification.
2. Provide a high quality patient focused medicine optimisation service, helping to provide a flexible and responsive service to patient needs.
3. Collaborate with the multidisciplinary team to deliver patient focused care to patients and ensure a timely supply of medicines, utilisation of patients' own medicines and counselling on medication.
4. Participate in medicines management, ward top-up dispensary and ward-based dispensing.
Job Responsibilities
1. To be responsible for the provision of a high quality patient focused discharge service to patients of the Trust and help ensure that service provision is flexible and responsive to patient needs.
2. To provide advice on the technical aspects of the service.
3. To check patients own medicines and those dispensed ready for discharge for their suitability for supply at discharge and highlight any shortfalls or queries with the patients prescribed/ not prescribed medications and the discharge prescription to the ward pharmacist.
4. To support the dispensary in the timely dispensing and supply of discharge medication.
5. To counsel patients as identified according to procedures and ensure that all discharge medicines are current.
