TheCellular Pathology Department is seeking an organised and patient focusedindividual to support in the management of diagnostic samples from patientson a cancer pathway on a 12-month fixed term basis.
Thedepartment provides a diagnostic histopathology and non-gynae cytologyservice, including fast track processing for patients who are suspected ofhaving cancer.
TheCancer Pathway Manager will provide comprehensive support to the CellularPathology Management Team through the co-ordination and management ofpatients on a cancer pathway that require diagnostic reports and will act asa single point of contact for the service, and as a liaison point betweenother CSUs, Multi-disciplinary Teams (MDTs), and the Corporate Cancer Team.
Thepost holder will support the service in its delivery of diagnostic reportsfor patients on a cancer pathway, ensuring diagnostic processing andreporting is provided in line with local and national targets.
Previousexperience of working within the heath care sector and knowledge of medicalterminology and patient pathways is advantageous.
Main duties of the job
Thisrole has been created to support the Cellular Pathology service in itsdelivery of diagnostic reports for patients on a cancer pathway, ensuringdiagnostic processing and reporting is provided in line with local andnational targets. As this is a new role within the service, the post holderwill need to be flexible to support the needs of the departments it evolvesover time. The Cancer Pathway Manager will provide comprehensive support tothe Cellular Pathology Management Team through the co-ordination andmanagement of patients on a cancer pathway that require diagnostic reports.
Thepost-holder act as a single point of contact for the service, and as aliaison point between other CSUs, Multi-disciplinary Teams (MDTs), and theCorporate Cancer Team.
Keyduties of the role include:
Attendance at weekly Patient AccessMeetings, providing analysis of patients on a fast track pathway, requiringdiagnostic reports
Fast track and Cancer waiting-listmanagement, including the tracking of patients and escalation of anybreaches.
Leading and supporting the servicein its service delivery and quality improvement initiatives for patients on acancer pathway.
Maintaining close relationshipswith the Corporate Cancer Team, acting as a key point of contact for theCellular Pathology department, and supporting in any associated activity.

Knowledge
Application of Trust Policies,practices and procedures associated with the role of Cancer Pathway Manager.

Understanding of definitions andreporting requirements of the cancer standards (DoH Cancer Wait TimesGuidance, Version 8, 2012).

7. CORE KNOWLEDGE AND UNDERSTANDING
TheCancer Pathway Manager will demonstrate understanding of local and nationalstandards/targets applicable to the area of service. This includes knowledgeof the NHS Cancer Plan and national cancer guidance. In addition, thepost-holder will have knowledge of key Trust information systems and theability to navigate these to track patients. Knowledge of the patientspathway relevant to the CSU specialty and knowledge and understanding ofapplicable medical terminology is required.
Knowledgeof administrative processes and procedures in an acute hospital setting,including awareness of the Caldicott Guidance and Data Protection andSecurity Act relating to patient information is also required.
PRINCIPAL DUTIES & AREAS OFRESPONSIBILITY
Communicationand Coordination
Thepost-holder will:
Communicate with relevantdepartments and organisations to ensure that the patients pathway is astimely as possible and all delays are avoided.
Communicate with key departmentsareas of the serviceto ensure all tests and investigations are completed in atimely manner and any issues are escalated.
Communicate information on thepatients progress on the care pathway to CSU managers and clinicians asappropriate.
Liaise and communicate cancerguidance and definitions to key staff within the Trust.
Develop positive workingrelationships with all members of the team, liaising in particular withmanagement, consultants, Pathologists and administrative staff.
To manage own workload, with theability to work independently and take the initiative where appropriate,planning and organising his/her own time efficiently and effectively to meetall requirements in a timely and accurate manner.
Information/ Data Collection
Thepost-holder will:
Develop and maintain a database toactively manage and record patient level detail.
Collect and record data on behalfof the consultants within the team, which can be manipulated to allow theextraction of data for audit and research purposes.
Using data gathered identify,develop and implement, through collaborative working with the cliniciansimprovements to the patient pathway and overall.
Collect and record patientinformation from Trust data recording systems (e.g. pathology, radiology,outpatients, bookings, PPM database etc. to ensure the patients progress canbe measured.
Trackingand Case Management
Thepost-holder will:
Work closely with consultants,Biomedical Scientists, clerical and other staff to deliver a coordinated,consistent management of patient cancer pathways.
Monitor the patients progress onthe pathway and check daily to ensure that escalation points are adhered to.
Utilise reports in the PPM Databaseto monitor patients and identify delays for action.
Liaise with other departments andtrusts to ensure that investigations, opinions and treatments are sought in atimely fashion.
Ensuring all new cancer patientsare tracked and highlighted for discussion at MDT meetings, when results etc.are available.
Escalationof Actions
Thepost-holder will:
Monitor patient pathways againstthe agreed timescales in accordance with the specified pathway.
Ensure that all relevantinformation is provided to the staff responsible for delivery of the cancerpathway.
Escalate those patients whoseprogress is likely to fall outside the agreed timescales.
COMMUNICATION& WORKING RELATIONSHIPS
Thepost holder will be required to establish, maintain and respect workingrelationships and communication networks associated with the post.Communication will include those organisations, individuals and institutionsthe post holder comes into contact with. Typically these will includeinternal networks associated with the specialty area as well as otherexternal contacts as appropriate. The post holder will be expected torepresent them and the service in a professional, courteous and respectfulmanner at all times.
11. SPECIAL WORKING CONDITIONS
Theremay be a requirement to support the delivery of service at other sites asnecessary. This could be a regular feature of the post or only an occasionalrequirement. This will be confirmed following discussion with line manager.
i) PHYSICAL EFFORT:
Advancedkeyboard skills and an expert knowledge of PPM are required.
Usinga variety of software applications to validate information demands accuracyand dexterity to ensure correct outcomes.
Frequentsitting in a restricted position whilst carrying out duties on a PC and overthe telephone.
VDUand highly accurate keyboard skills for prolonged periods of time.
ii) MENTAL EFFORT:
Frequentmental effort/concentration required in relation to coordination ofprocedures and appointments to facilitate the patients clinical pathwayUnpredictable workload, responding to changing circumstances(increase/decrease in demand/capacity, prioritisation of workload)
Robustverbal and written communication skills required at all times.
iii)EMOTIONAL EFFORT:
Trackingof individual patients within tight timescales can be stressful and theresulting action can involve liaising with un-cooperative members of staff.
Therewill be frequent exposure to emotional circumstances and difficult situationsincluding death and dying, distressed patients, families and relativescarers.

iv)WORKING CONDITIONS:
Thereare no anticipated problems with extremes of environment. The post-holderwill be situated in an open plan office.
Willuse VDU equipment for most of the day, and will use the telephone on a dailybasis.
