To work collaborately with other senior nurses across LPT as appropriate and promote the role of their speciality nursing to all stakeholders., The successful candidate will clinically lead the Continuing Care Team within the Diana Service, in line with policy and procedure to enable the safe management of specific health needs and appropriate delegation of aspects of care, using necessary documentation for Leicestershire Partnership Trust.Supportingthe Continuing Care Team in delivering packages of care in the community in a safe and effective way, working alongside the children/young people and their families, acute hospitals, commissioners, and the wider MDT.
The post holder willsupport other members of the Diana Leadership team in the delivery of effective leadership and communication, attendance at monthly meetings providing expertise and information.Alongside the Diana Service Clinical Leads develop the annual training programme for the workforce across the Diana team. Ensure that staffs are supported to achieve service expectations for clinical competence.Actively developing the service to ensure the health needs of children in the community are met and highlight any gaps to management and commissioners.
Ensure and support the current nursing workforce to meet current service demand in relation to clinical competence.Proactively contribute to the longer-term work force development to ensure that the service can meet future clinical demands.To participate in the on-call service provided to staff and end of life children and young people within LLR., working patterns in line with our flexible working policy.
