Job overview
An exciting opportunity has arisen for an experienced Health Visitor to join the new Pre and Post Birth Multiagency Team. The team will work jointly with Children's Social Care to offer an enhanced health visiting service throughout the antenatal period until 1yr post birth. The team will be co-located within Lynton House in Ilford to ensure good multi agency working.
Main duties of the job
The post holder will achieve this by being an integral part of the ‘Pre and Post Birth Multi Agency Team’ within Children’s Social Care. This team will include a mix of social workers, health practitioners, family support workers, whilst working alongside third sector agencies. Working with the multidisciplinary team to deliver an enhanced health visiting experience and delivering the Healthy Child Programme, aiming to improve antenatal health and child health up to the child’s first birthday. The post holder will develop supportive relationships with the parents to improve outcomes for children and families with additional vulnerabilities. This is a demanding role requiring a high level of professional skills and practice, and the ability to work as part of a wider multi agency team.
Detailed job description and main responsibilities
· To act as health’s champion within the pre and post birth team, bringing expert operational knowledge and skills to the team surrounding the impact of safeguarding on children’s health and development.
· To be responsible for a caseload of children and families with additional vulnerabilities who are being supported within the wider Family Help team in Children’s Social Care.
· To deliver the Healthy Child Programme to the post holder’s caseload of children and families.
· To take a lead professional role when required.
· To undertake home visits in accordance with local guidance and policy.
· To work in collaboration with the multi agency team.
· To be responsible for organising and prioritising own and others workload in the day-to-day allocation of work.
· To deputise when required in the team managers absence and delegate appropriately to junior staff.
· To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling.
· To be responsible for providing accurate records of information required by the Trust for audit purposes.
· To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.
· To supervise pre-registration and post-registration SCPHN students.
· To employ professional curiosity in your day-to-day role and act upon any findings appropriately.
· Visit clients across a wide geographical area.
· Complete full, accurate and contemporaneous records in line with trust policy and NMC guidance.
· To interpret health information accurately to support the safeguarding process and offer relevant advice and guidance to the wider multi agency team.
Person specification
Qualifications
* Specialist Community Public Health Nurse – Health visitor
* To hold a relevant qualification at masters level or equivalent experience
* Safeguarding supervisor trained
Experience
* Minimum of 2 years post SCPHN qualification, working actively as a Health Visitor
* Minimum of 2 years experience in working with vulnerable families
* Experience in collecting and analysing data
Knowledge
* An awareness of adverse childhood experiences
* Awareness of Trauma Informed Practice
* An awareness of substance misuse
Skills
* To be able to work with families using trauma informed practice
* To be able to work with families using a strength-based approach, using SMART goals
* To be able to communicate effectively with families, children and other professionals
