We are looking to recruit a caring, flexible and enthusiastic team member to join the Macmillan Upper Gastrointestinal and Hepatobiliary Clinical Nurse Specialist Team. The job involves ensuring the coordination of patient pathways across a number of disease types (oesophageal, gastric, hepatic and pancreatic cancers) ensuring the timely organisation of appointments, investigations and MDT discussions. The job will also involve conducting Holistic Needs Assessments to explore the individual needs of the patient and their family. Training will be given to support this learning. The role also involves sensitively handling telephone enquiries from patients and their families so the ability to communicate with compassion and professionalism is essential.
Main duties of the job
1. Triaging patient enquiries and providing follow-up support to Upper GI and HPB Cancer patients to ensure their care pathway is coordinated and taking place as planned. This will involve liaising with colleagues within the Trust and Leeds Teaching Hospitals.
2. Ensuring effective communication with the Clinical Nurse Specialist Team and highlighting issues for their attention.
3. Providing administrative support to the Clinical Nurse Specialist Team ensuring that patient pathways are tracked and that investigations and appointments are made in a timely manner and in line with current national targets.
4. Undertaking Holistic Needs Assessments to ensure the delivery of personalised care which reflects the needs and values of the patient and their family.
5. Responding to patient's needs and implementing appropriate referrals to further services.
6. Answering telephone enquiries where possible and escalating to nursing team where required.
You will need to have a professional manner, excellent communication skills and an empathetic nature. Attention to detail, interpersonal skills and an adaptable approach to work are essential as are literacy and IT skills.

Qualifications
* NVQ level 4 in related Area
* 5 GCSE's including Maths and English at grade C or equivalent
Experience
* Significant experience of working with patients who have cancer
* Experience of working with Multi-professional teams
* Experience of telephone triage
* Administration support for clinics
Knowledge
* Insight of cancer pathways
* Insight of the Impact of cancer on patients and carers
Skills and Aptitudes
* Ability to manage low risk, routine case load of patients and plan own workload
* Ability to collate complex information
