An opportunity has arisen for a dynamic and self-motivated nurse to join the chronic kidney disease (CKD) team at ESNEFT on a fixed term basis of 24 months currently. This is an exciting role working within an established secondary care team, to develop our service to provide care also in an outreach setting, in collaboration with primary care teams. This new service will provide a high level of support delivering care to both patients and their families closer to home across East Suffolk and North Essex. This service will be compliant with NICE Guidance NG203 (Chronic Kidney Disease: assessment and management) and will facilitate delivery of novel treatments for patients with CKD (SGLT2 inhibitors). Internal applicants currently employed by ESNEFT will be offered on a secondment basis only. Main duties of the job You will be required to work closely with primary care partners across East Suffolk and North Essex, delivering clinics in a number of primary care settings. Attendance at MDT meetings will be essential also. We are looking to recruit a Registered Nurse who has demonstrable experience in the renal setting, as there is a strong commitment to education and health promotion by adhering to regional and national strategies and guidelines with emphasis on advanced practice within this role.
