An exciting opportunity has arisen for a highly motivated, enthusiastic, experienced Advanced Nurse Practitioner to join our friendly General Practice in Kettering.
The post holder will be an experienced Nurse Practitioner and independent prescriber who, acting within their professional boundaries will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice.
You will work closely alongside our existing 2 ANPs.
Main duties of the job
We expect that you will be able to work independently and manage your own workload. You will need excellent communication skills, able to meet targets and to be able to liaise with patients, primary care staff, community pharmacists and other partners.
Must have experience of clinical triage, home visits and be an independent prescriber.
Key Responsibilities
* Works autonomously and is accountable for own professional actions.
* Assess, diagnose, plan and implement treatment/interventions and care for registered patients.
* Buddy / Support for on-call GP and partners.
* Refers patients to other medical specialities for assessment if deemed appropriate.
* Diagnose and manage both acute and chronic conditions.
* Prescribe and review medication for therapeutic effectiveness in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
* Works with patients to support compliance with prescribed treatments.
* Communicates highly sensitive condition-related information to patients, relatives, and carers as necessary.
* Initiates and manages those patients with suspected or confirmed chronic disease in line with the disease management agenda i.e. QOF or IIF.
* Undertake annual review of patients.
* Checking and actioning Tasks, lab reports and clinical correspondence.
* You will support the weekly ward round to our designated nursing home either virtually or F2F to support residential patients.

Job responsibilities
The Advanced Nurse Practitioner (ANP) will deliver a high standard of patient care, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.
* Will independently prescribe appropriate medication, evaluate, or refer to other specialists if necessary.
* Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
* Support On-call Clinician.
* Implement and evaluate individual specialised treatment plans for chronic disease patients.
* Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to the patients' health.
* Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
* Undertake the collection of pathological specimens.
* Request pathology / radiology services as necessary.
* Process and interpret pathology and other test results as required.
* Sign electronic prescriptions.
* Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
* Maintain accurate clinical records in conjunction with extant legislation.
* Ensure SNOMED CT codes are used effectively.
* Carry out home visits.
* Prioritise health issues and intervene appropriately.
* Support the team in dealing with clinical emergencies.
* Recognise, assess, and refer patients presenting with mental health needs.
* Be an extended and supplementary prescriber.
* Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice), reviewing annually as required.
* Contribute to practice targets (QOF, etc.), complying with local and regional guidance.
* Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.).
* Support the clinical team with all safeguarding matters in accordance with local and national policies.
* Deliver opportunistic health promotion where appropriate.
* Attend local training events.
* Attend and participate in practice meetings.

Qualifications
* Adult Nursing Qualification.
* Relevant Advanced Nursing Qualifications.
* NMC Full Registration.
* Experience in clinical triage.
* Full UK driving Licence.
* Experience in clinical triage within General Practice.
* Experience working with care homes.
