Job summary We are looking to recruit a highly motivated and innovative, with experience of working in a clinical setting Health Care Assistant to work within the Rotherham area, delivering Health Checks into external workplaces, GP practices and screening the health of employees/patients across the borough of Rotherham. You will also be expected to support employee/patients by giving health advice face to face as well as signposting and cross-referring to other support services. You will be expected to work within the team, but also independently in external workplaces to carry out this role, following specific policies, guidelines and protocols. There will be lone working involved. Your role will be delivered across various workplaces/GP practices in Rotherham therefore access to a suitable vehicle and the ability to travel to a range of venues and settings is essential. We are looking for someone who has experience of delivering NHS Health Checks (albeit training can be given) and is able to evidence an understanding of Public Health initiatives and programmes Main duties of the job Main duties of the job Completion of point of care health check and questionnaire incorporating BMI, BP, GPPAQ, AUDIT-C, Cholesterol/HDL and CVD for employees/patients in their place of work or GP practice. You will be making contact with employees face to face, so effective communication skills and professionalism are essential. You will be signposting and cross-referring to other support services where applicable. To deliver at point of care advice around improving cardiovascular health and providing easy to understand analysis of the outputs from the health check. Requesting feedback from the employees. Questionnaires and health check data will be completed online, full training on the software will be delivered, however basic computer skills be required. The output from this will often feed into SystmOne, where it does not, a personalised health report will be provided to the employee.
