Barnet Council is seeking a dedicated Social Worker to join the Adults Early Intervention Team. Our strengths-based approach supports residents in maintaining their independence and well-being, ensuring they receive the care they need in their community.
About the Role
* Facilitate timely, safe hospital discharges and support individuals in their transition back to the community.
* Conduct complex assessments and safeguarding enquiries, ensuring proportionate and effective support plans.
* Work collaboratively with adults, families, carers, health professionals, and partner organisations.
* Undertake Mental Capacity Act assessments in line with statutory guidance.

About You
* Qualified Social Worker, registered with Social Work England.
* Experience working with vulnerable adults in a social work setting.
* Strong assessment and report-writing skills.
* A commitment to person-centred and strengths-based practice.
