Shropshire Community Health Trust is offering an exciting opportunity for motivated individuals to work within the Integrated Health and Social Care Care Transfer Team at the Shrewsbury and Telford Hospital Trust. The team is expanding, and this is an ideal opportunity to be involved in the transformation of the service and work with the existing skilled team in the specialist field of Complex Discharge and Patient Flow.
The service operates 8am - 6pm, 7 days a week, this post includes bank holidays and weekend working as part of the team rota.

Main duties of the job
* To be responsible for ensuring efficient and timely flow of patients.
* Accountable for preparation and distribution of accurate reports. Internal and External Bed States throughout the day from information provided by all four Community Hospitals and the pathway 2 beds within the independent sector beds as well as System Partner beds held with Local Authority and Neighbouring trusts.
* To liaise with the Inpatient wards for information regarding out of hours admissions and discharges.
* To triage referrals using agreed criteria and ascertain priority of patient referrals.
* To work with Neighbouring Acute and Community Trusts to repatriate Shropshire patients and to keep an active caseload/waiting list.
* To attend the Repatriation weekly meetings and provide updates on current caseload. To Ensure waiting list and RIO is updated.
* To work autonomously within the Capacity Hub Standard Operating Procedures, taking responsibility for bed availability and bed allocation and for allocating patients to appropriate beds using sound knowledge of the admission process and Trust and Government targets.
* Liaise appropriately with all members of the Divisional and Operational Teams to ensure optimum use of bed capacity within the Trust.

Person Specification
Training and Qualifications
* GCSE level Maths and English or equivalent
* Word Text Processing (or equivalent)
* NVQ Level 3 in Administration, or Level 3 NVQ in Health Care Related topic or equivalent
Experience
* Experience of working within an office environment
* Dealing with the public face to face and on the phone
* Working with hospital computer systems
Knowledge and Skills
* Articulate with well-developed communication (verbal and written) and interpersonal skills.
* Good organisational skills; self-starter and able to use initiative appropriately with only little supervision.
* Ability to prioritise and work on own initiative.
* Computer literate with experience and knowledge of Microsoft Office - Word, Excel, PowerPoint and Outlook.
* Knowledge of the Discharge Pathways.
* Knowledge of Community services and resources.
* Demonstrates accuracy and attention to detail.

£24,071 to £25,674 a year pro rata per annum.
