Directorate: Social Care Health Integration&Wellbeing Section: Review Team Grade: Level 10 Actual Salary: £36,648 - £39,186 Hours: 37 This advert is open to both Internal and External applicants. A fantastic opportunity has arisenfor a Social Worker to join the newly established Review Team. As a Social Worker you willundertake a range of assessments and reviews and manage safeguarding concerns whilstensuring a needs led approach, ensuring a focus on community assets in partnership withindividuals, carers and other professionals and where applicable arrange appropriate supportpackages that address the eligible social care needs identified. Responsibilities will routinelyinclude Social Care assessments for adults and carers, reviews, care and support planning andcontinuing health care decision support tools. This will require accurately maintaining clientrecords and ensuring that these records are logged correctly on to the departments IT systemalong with strong report writing and communication skills. You will also be supported withregular supervision and performance management sessions. Along with a relevant qualificationyou should have relevant knowledge and experience in Adult Social Care either in a formal orinformal capacity and be able to fully meet the requirementsof the post and cover the wholegeographic area of the City either by possession of a current and valid driving licence or byaccess to public or other transport.
