Job responsibilities JOB SUMMARY To work with the existing Sarcoma MDT Coordinator to ensure all areas of the MDT are covered on a daily and weekly basis. To carry out administrative duties in relation to the team meetings and ensuring appropriate records are maintained The post holder will be responsible for inputting and maintaining a robust data collection system to provide clinical and cancer waiting times information and to produce reports as required by members of the MDT. Working under own initiative and organising the day to day tasks of the role, liaising closely with the Tumour Site Lead, members of the MDT, Clinicians, Directorate Managers and the Cancer Management Team. The post holder will be an integral part of the Trusts achievements of Local and National Cancer Targets. KEY JOB RESPONSIBILITIES To communicate with a range people including clinicians, management, and other departments and outside agencies that could be complex in nature. This may require the transfer of sensitive information. To liaise with all disciplines involved in the specialist cancer pathway to ensure the timely management of patients. To co-ordinate with the Nottingham Cancer Centre and any regional centre regarding local and tertiary referrals to the MDT. To co-ordinate with Nottingham Cancer Centre to prevent breach of cancer targets and work to the Local and National Cancer targets Create a patient pathway spreadsheet which establishes a prompt timeline for staging investigations and any interim procedures To co-ordinate correspondence and actions recommended by other MDTs regarding the cancer specific pathway patients Responsible for the tracking and retrieval of patient notes as necessary Responsible for monitoring and inputting information to form an accurate report for the weekly cancer PTL for the Operational team. Administration of Team Meeting To provide administrative facilitation of cancer speciality meetings including preparing, organising and circulating agendas, and recording the outcome of MDT meetings. To assist and supervise the collation of all relevant documentation for team meetings, to include patient notes (from wards and departments), x-rays, test/investigation results, letters, etc. To identify and be involved with the co-ordinating appropriate patient referrals for discussion at the MDT team meetings using defined protocols/guidelines and liaising closely with the Tumour Site Lead Clinicians. To inspect and approve the MDT list prior to circulation to ensure data is complete and to avoid unnecessary repetition. To confirm MDT decisions to referring clinicians and primary care. To input MDT minutes onto Notis accurately and in a prompt and timely manner. To confirm MDT decisions to referring clinicians and primary care. Create a CRC MDT referral and response pathway on Notis. To maintain an attendance record of core and non-core members of the CRC MDT as part of Peer review submission. To maintain rota with consideration for A/L. on call nights etc. and to maintain record for review. To co-ordinate with secretaries, service managers and administration managers regarding MDT actions, available theatre sessions and clinic appointments etc. Create correspondence as appropriate from pre-defined templates to other multi-disciplinary teams (MDTs), GPs, PCT & Public Health. The Coordinator will have to deal with and be exposed, indirectly, to sensitive and often emotional circumstances as they follow the Cancer Patient through the treatment pathways. Data Collection Create & Input methods of data collection e.g. database/spreadsheet to support the collection and maintenance of data for audit purposes Accurate data entry, analysis and checking are required to ensure reports are produced precisely to form part of the reporting systems required by the Trust to achieve National Targets. Identify areas of performance where there is variance and report to lead clinician and operational management team. To assist the Lead clinician with the 6 monthly audit meeting programme. To arrange 6 monthly audit meetings and other MDT meetings as required, locating suitable venues, informing attendees and completing minutes. Assist the Lead clinician in dealing with incoming MDT service correspondence and facilitating prompt action on results/reports. Assist the Lead clinician with the production of MDT induction pack and standard operating procedures for medical staff where appropriate, working from defined guidelines pertaining to the CRC MDT or patients cancer pathway. Training and Support. To provide cover for other MDT Coordinators within the CAS Directorate during periods of absence. This will mean covering the duties of other MDT meetings.
