What you will do: - Conduct comprehensive assessments such as Community Care and Best Interest Assessments to create effective care solutions. - Manage discharges from out-of-area hospitals, recovery hubs, and temporary placements, as well as review care in people's own homes. - Undertake safeguarding investigations, adhering to multi-agency procedures. - Perform risk assessments and develop strategies to promote independence and early intervention. - Establish and nurture person-centred relationships with service users, families, and carers for informed decision-making. - Collaborate with courts to secure and implement court orders when necessary. - Participate in specific casework across teams and support service development. - Engage in the Practice Educator Level 2 role and the Approved Mental Health Practitioner (AMHP) rota if qualified. - Handle office duty responsibilities as required.
