Responsibilities
Conduct comprehensive assessments of clients' needs and circumstances.
Design, implement, and monitor individualized care plans to address the needs identified.
Provide counseling and support to individuals and families facing various challenges.
Coordinate and collaborate with multidisciplinary teams and external agencies for effective service delivery.
Advocate for clients' rights and access to necessary resources and services.
Maintain accurate and up-to-date records of client interactions and progress notes.
Engage in continuous professional development and training related to social work practices and policies.
Requirements
A recognized degree in Social Work or equivalent qualification.
Registration with the Social Care Wales as a qualified social worker.
Demonstrated understanding of social work principles, ethics, and legislation.
Experience working with diverse populations and in complex case management.
Strong communication and interpersonal skills to build rapport with clients.
Ability to work collaboratively in a multidisciplinary team setting.
Commitment to ongoing professional development and adherence to the All Wales Pledge.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £57.59
Location: Swindon, Wiltshire
