We are seeking a dynamic, self-motivated RN Case Manager - Palliative/Hospice Care, who thrives in a fast-paced environment and supports our mission.
POSITION PURPOSE/SUMMARY: Provides skilled nursing assessments, planning, and care in compliance with the interdisciplinary team plan of care, under the supervision of the Director of Palliative Care and the direction of the physician according to the policies and procedures of Hospice of San Joaquin and the Palliative Care program.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Provides Palliative education/information, emotional support, and community resource information to program participants who have advanced chronic or life-limiting illness via telephone communication and/or perform home/facility visits.
2. Completes the initial nursing assessment using clinical assessment skills; identifies the physical, emotional, physiological, spiritual, and practical needs of the patient and family.
3. Develops treatment plans based on identified needs, reflecting a problem-solving approach with measurable defined outcomes.
4. Utilizes Hospice of San Joaquin electronic medical records systems for patient documentation and maintains up-to-date patient records.
5. Maintains up-to-date patient records in accordance with Medicare, medical, State home health agency, and JCAHO standards.
6. Updates the plan of care using clinical assessment skills specific to the Palliative Care patient population and provides suggestions to the IDT for action and follow-up by various team members.
7. Ensures HIPAA compliance.
8. Provides referral outcome information to the managing physician.
9. Provides educational and informational visits to referral sources/agencies.
10. Maintains a professional appearance and exhibits appropriate behavior with customers/participants and staff.
11. Uses assessment skills to identify the needs of patients and families, providing support based on identified needs.
12. Uses expert knowledge of hospice nursing, crisis intervention, and family dynamics to determine the appropriate level of care.
13. Collaborates with pharmacy, DME, and other healthcare providers to provide appropriate care.
14. Functions as an on-call nurse, determining the necessity of home visits.
15. Follows universal precautions in delivering patient care.
16. Demonstrates creativity in meeting patient/family needs in the home environment.
17. Supervises and documents the Home Health Aide plan of care.
18. Follows policies and procedures of Hospice of San Joaquin and Palliative Care Program.
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily and comply with the Hospice of San Joaquin Personnel Policy and Procedures Handbook.
EDUCATION and EXPERIENCE: Graduate of an accredited school of nursing in the State of California as a Registered Nurse. Three years of recent clinical experience. Computer skills and the ability to learn the clinical computer program system are required.
LANGUAGE SKILLS: Excellent verbal and written communication skills. Ability to analyze and interpret general business periodicals, professional journals, technical procedures, or government regulations.
MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
REASONING ABILITY: Ability to apply common sense understanding to carry out instructions furnished in written or oral form.
CERTIFICATES, LICENSES, REGISTRATIONS: Possess reliable transportation and a valid California driver’s license. License to practice as a registered nurse in California.
