This is an exciting opportunity for the ‘right’ person who wants to work supporting people by coordinating their health and social care services. Based in a GP Practice, the Care Coordinator will be involved in supporting the clinical team to proactively identify and work with patients, including the frail/elderly and those with long term conditions, to provide proactive, person-centred planning, helping coordinate care by bringing together the different specialists who help that individual might need.
This new role has been developed to support Oakmeadow delivering high quality care to patients. To support our Reception and Administration Team using System One and other third-party software.
Your duties may be modified from time to time to suit the needs of the business. In order for the Practice to operate effectively, you are expected to be flexible in your approach to work and to undertake different tasks consistent with your ability and training when required.
Care Coordinators will:
* Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids.
* Bring together all of a person’s identified care and support needs and explore their options to meet these into a single personalised care and support plan.
* Provide administrative support and services to all members of the practice team.
* Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.
* Cover Reception Team, Administration Team and Secretary when needed to include sickness and annual leave.
* Update IT Systems and manage our Website and Facebook.
* Help people to manage their needs, answering their queries and supporting them to make appointments.
* Ensure that people have good quality information to help them make choices about their care.
* Liaise with external organisations where necessary.
* General Administration and note taking as appropriate.
* First port of contact for our Patient Participation Group (PPG).
