Main duties of the job CLINICAL To provide a high standard of nursing care to a specified client group. To fulfil the role of Care Coordinator for a caseload of patients residing in the community. To monitor restrictions under the terms of conditional discharge as set out by the Ministry of Justice. To fulfil the requirements of MAPPA for patients that this is required. Participate fully in the MAPPA process To provide continuous review of a caseload, reporting to MDT's and the FOS Manager, utilising collaborative care planning. To attend clinical meetings at secure in patient units across Prospect LPC, outpatient department and any other areas as necessary. To observe confidentiality in the care of patients and their case records. To administer medication as prescribed to individual patients. To provide support for patients at home or in supported accommodation. To regularly attend the outpatient department for the assessment and follow up of patients.
