This newly created position is an exciting addition to our population & health equity work. This role is pivotal in taking our tobacco dependency support services into community settings, reaching people using our services, their families and carers, and our colleagues, in the community. The role sits alongside our tobacco dependency support services already provided in our inpatient settings, so you'll have a team of peers to connect with. You will, however, be skilled at working alone as the majority of your time will be supporting people in community settings.
This role is funded for an initial 2-year period. With the right outcomes and impact, we'd hope to extend that timeframe.

Main duties of the job
We're looking for an exceptional colleague who can deliver tobacco dependency support services to our community-based population, including service users, their families and carers, and Trust colleagues.
This is a new role within the Trust, funded for an initial period of two years, so we're looking for somebody who can hit the ground running, who can, for example:
1. Develop and implement a project plan.
2. Interpret complex population health data.
3. Identify, recruit, and coordinate a team of Trust Smokefree Champions within our teams.
This role is the ideal next step for an experienced Stop Smoking Practitioner looking to lead and develop a new and innovative community-based service. You will have autonomy to make the role your own, within an agreed scope, and be able to demonstrate your impact. We're looking for a leader who can pick up and run with our aspiration for healthier communities.
Knowledge of the NHS structure and tobacco dependency support services is a must. Ideally, you will have experience of working with people in the community, with demonstrable evidence of engaging people who, traditionally, may not access proactive healthcare solutions.

Person Specification
Qualifications
* Educated to degree level.
* Programme and/or project management experience.
* Currently registered as a NCSCT Stop Smoking Practitioner (or willingness to complete this within 3 months of taking up this role).
* Business analysis and/or quality improvement (QI) experience and skills.
Experience
* Previous NHS experience, working in a management role where negotiation and influencing skills were a key part of the job.
* Proven experience and successful track record of defining and implementing change, including an advocate for engaging patients and hearing patients' voices to help shape policy and development.
* Confident leader capable of working at all levels of management within the NHS from executive leaders to teams delivering services and treatment.
* Understanding and experience of multi-disciplinary working and liaison with other services.
* Smokefree knowledge or prior experience.
* Previous experience of working with patients with a serious mental illness and/or chronic tobacco dependence.
* Experience of data analysis and audit and knowledge of how this information can support practice.
