Job overview
As band 6 Sister/Charge Nurse you will be expected to deputise for the lead nurse on a regular basis to ensure the function of the Discharge Lounge is maintained to deliver a smooth transition from hospital to home or other specified destination for all patients.
Expedition of discharge includes but is not limited to: the collection of ward patients throughout the day, frequent communication with ED and Emergency Floor senior staff to identify patients suitable for the discharge lounge and attending morning board rounds.
Promote the ethos of the discharge lounge to support all clinical colleagues, whether in expediting early discharges, or at times of low demand by assisting short-staffed wards in delivering direct patient care including hygiene and personal care.
Main duties of the job
1. To deputise for the band 7 lead nurse to manage the day-to-day running of the discharge lounge. To assist with off duty, sickness reviews, updating policies and procedures.
2. To assist flow throughout the hospital to escort and transfer patients from the emergency floor to clinical wards.
3. Supporting service delivery and ensuring full cover between the working hours of 07:30 – 20:00.
4. To ensure the discharge lounge runs effectively delivering high standards of care.
Detailed job description and main responsibilities
1. Comply with Trust and Departmental policies and procedures e.g. Health and Safety at Work Act (1974), Manual Handling, clinical equipment competency, risk management, Data Protection Acts and patient confidentiality at all times. This will include demonstrating compliance with the terms of all safeguarding policies and processes relevant to the safeguarding of adults and children in the care of UHSx and to undergo regular mandatory training concerned with safeguarding matters.
2. Maintain accurate and contemporaneous patient records.
3. Demonstrate an understanding of research and evidence-based practice, and ability to participate as required in audit.
4. Knowledge of clinical governance, policy and procedure.
5. To adhere to all Trust policies and procedures and professional code of conduct.
6. Ensure a smart, professional image that enhances the public perception of the Trust and the professionalism of its staff is portrayed.
7. To lead and deliver safe person-centred care within a challenging and changing clinical environment, using equipment and resources safely and as appropriate. This will include the management of the Discharge Lounge on a regular basis and ensuring staff skill mix remains at an appropriate and safe level.
8. To support line manager in recruitment, retention, and performance-related issues, including being regularly responsible for the supervision and mentorship for unqualified/qualified staff and students.
9. Support line manager for clinical area in the management of budgetary control and resource management including duty rotas and skill mix requirements.
10. Actively participate in the multidisciplinary team (MDT), attending MDT meetings and liaising with/ advising other members of the MDT when necessary.
11. Recognises the contributions of others and contributes own knowledge and skills within the multi-disciplinary team.
12. Understand the need to be financially aware: reduce waste and work with the team to support financial sustainability whilst maintaining quality services for patients.
13. Contribute to clinical governance by active participation in service initiatives, health and safety and risk management.
14. To participate in developing systems for assessing the views of service users regarding the quality of services and for involving patients’ relatives/ representatives in the planning and development of services.

* Experience in leading a team and HR processes

* Excellent communication skills, both verbal and written
* Effective IT skills, including the use of Careflow, health roster and Affinity
* Can confidently communicate with family, patient transport, volunteers to convey important information

* Knowledge of the wards of Worthing hospital, their specialities and locations
* Experience in creating and updating policies and SOP's
* Able to provide patients with high standards of care including delivering medications
* Demonstrate a clear understanding of safeguarding policies
* Participate in audits/ projects to improve patient care
* Evidence of being involved in an improvement project
