Job overview
We are seeking a Band 6 Mental Health Transition Practitioner to support young adults aged 18-25 during their transition into the CMHT/Recovery Team, connecting them with appropriate pathways and interventions.
Main duties
The role involves engaging with young adults, carers, and referrers, supporting informed decision-making, and promoting NICE transition standards. The practitioner will be part of the MDT, working closely with children’s services and external agencies supporting 18-25 transition across primary care and VCFSE sectors.
Key responsibilities
* Support transition planning from CYP services to adult mental health services, starting from school year 9.
* Develop and oversee coordinated transition plans and review meetings.
* Ensure person-centred, strength-based support focusing on individual goals.
* Promote flexible, needs-based transition processes.
* Collaborate across services to ensure seamless transfer and continuity of care.
Qualifications and experience
* Degree-level training or equivalent.
* Professional registration (e.g., NMC, Social Work England, HCPC).
* At least one year of post-registration experience, with autonomous practice skills.
* Experience in mental health care, care planning, risk management, and working within MDTs.
Skills and values
* Knowledge of relevant legislation (Mental Health Act, Mental Capacity Act).
* Ability to work independently and collaboratively.
* Engagement skills with service users and carers.
* Commitment to high professional standards, accountability, and supportive practice.
