Highly Specialist Pharmacist - Homecare and Medicines Optimisation

We are looking for a Pharmacist with experience in Homecare and medicines optimisation to work at Bolton NHS Foundation Trust.
This is an exciting opportunity for a pharmacist to join the wonderful friendly pharmacy team at Bolton NHS Foundation Trust.
We are looking for a specialised Band 8a pharmacist who will play a significant role in leading the homecare service, and contributing to the medicines optimisation agenda at Royal Bolton Hospital. This is an exciting new role for the department and are looking for a forward thinking candidate to help drive the service.
We want all our staff to have the ability to reach their full potential and share with others the great work they do. Bolton has electronic prescribing throughout the hospital and is a leader in digital transformation projects.
· To lead the homecare and medicines optimisation pharmacy team, to ensure high cost drugs are managed safely and effectively.
· To support specialities and areas that use high cost medicines, working in partnership with commissioners to ensure the most cost effective use of these medicines in line with national and local guidance.
· Lead and manage the homecare pharmacy team, responsible for setting up new services and reviewing homecare provider performance and ensure patients are receiving safe and effective care.
• Provide clinical support as a senior Pharmacist to the dispensary team and other members of the pharmacy team as required.
