An exciting opportunity is available to contribute to the provision of Respiratory and TB Services in Luton community services to meet the needs of people with respiratory conditions, including COPD, Asthma, ILD, Bronchiectasis and TB who require community support.
This role will include:
1. To provide specialist nursing advice for both primary and secondary care and work to prevent TB by vaccination, education and the monitoring of patients receiving care and treatment. In conjunction with UK Health Security Agency Staff, primary and secondary care this post implements preventative and infection control measures according to agreed protocols.
2. To support the delivery of education, support and training for service user, community nursing teams and primary care.
3. To work with colleagues to provide high quality, evidence based, integrated services for adults and older people.
4. To work towards increasing community access to support a reduction in health inequalities and increase quality of life for patients with respiratory conditions.
5. Clinical responsibility for the assessment, planning and co-ordination of care for a caseload of patients, to avoid unnecessary hospital admissions and prevention of acute exacerbations.
Main duties of the job
1. To support the vision, purpose and objectives of the Community Respiratory and TB Services.
2. To support the Service manager/Lead Nurse to ensure services achieve their quality, performance, productivity, financial and contracting requirements.
3. Contribute to the continued development of the Services through effective collaboration with peers and partners across all community and specialist units.
4. Ensure compliance with agreed governance systems across the service.
5. To be accountable for the management of patients in your care, to ensure a high standard of evidence based clinical care is maintained.
6. Organise investigative procedures in accordance with local and national TB recommendations.
7. Support the development of integrated patient pathways, including delivering care in clinics and domiciliary settings.
8. Ensure all electronic based patient records are kept up to date in line with the Trust's record keeping policy.
9. Work within CCS policy and service specific Standard Operating Procedures.
10. To work in partnership with patients encouraging appropriate use of the resource, including social care agencies and the voluntary sector, raising the profile of respiratory needs across Luton, considering the diversity of population and the promotion of respiratory health.
11. Work with colleagues in the Acute Trust and Primary Care to identify patients who need the support of the multi-disciplinary team to prevent frequent hospital admissions.

The post holder will maintain verbal and written communication with patients relatives and carers and be able to maintain effective communication with external agencies including acute sector, primary care and the voluntary services.
Communicating sensitive condition related information to patients and relatives requiring empathy and reassurance.
To be proactive in raising awareness of TB and act as a resource for other health professionals and the wider community. The post holder will be expected to participate in relevant meetings and undertake teaching sessions to staff and patient groups.

Maintain own professional development and requirement to take part in appraisal and KSF process.

Clinical and Practice Governance
Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.
Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken regarding the Trust Information Governance and Information Security policies.
The post holder must adhere to the Trust risk assessment and risk management processes.

It is a condition of your employment that you are currently registered with the NMC, and it is your responsibility to maintain your professional registration.
Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

Provide patients and their families /carers with information on standards they should expect from the service and ensure the service has mechanisms to capture user and carer experience.
Participate in clinical supervision on a regular basis and to take part activities to monitor compliance with CQC essential standards and take remedial action where appropriate.

Qualifications
Essential
* Recognised qualification at Degree level in Adult Nursing.
* Relevant post registration Respiratory/TB course.
* Nurse Prescribing qualification or willing to undertake training.
* NMC Registered.
Experience
Essential
* Experience of partnership and multi-disciplinary working. Evidence of an understanding of local and national policy development for Respiratory /TB services.
* Understanding of working within a community setting.
* Competent in caseload management.
* Evidence of post graduate experience caring for patients with a range of Respiratory conditions or those with TB.
Desirable
* Experience of working with patients in a Pulmonary Rehabilitation setting.
* Knowledge of Immunisation programmes.
Knowledge and Skills
