We are currently recruiting for a Healthcare Assistant to join our community Paignton Health and Well-Being Team. To work as a Healthcare Assistant within an integrated team, you will work across the following services: Community Nursing, Intermediate Care, and Community Matrons.

Main duties of the job
You will deliver high-quality care to the housebound patients of Paignton and work as part of a multi-disciplinary team in partnership with primary care and secondary care.

Detailed job description and main responsibilities
* Work as a member of a specialist team
* Document clearly and concisely in patient notes utilising Trust wound care documentation
* Communicate using a variety of resources – face to face, telephone, and email in a timely manner
* Communicate with patients, their families, and friends
* Communicate with other specialist surgical and medical services; GPs and other relevant Health and Social Care professionals
* Take part in team meetings, encouraging effective communication between other staff
* Escalate any incidents or near misses immediately to the appropriate team members, and complete reporting via Datix.
* Work within the agreed Clinical Governance Framework, incorporating audit, clinical effectiveness, and incident reporting.
* Work within the limits of own competence and responsibility within the team and organisation, as identified by the appropriate accountable professional.
* Educate and support patients/carers in the use of equipment in accordance with Local Guidelines and within own competence.
* Encourage, enable, and support individuals, wherever possible, to gain or maintain confidence, independence, and participation in social, recreational, and community activities.
* Attend mandatory and other appropriate training courses to meet personal development objectives and assist in the training of others as directed.
* Work independently using a wide range of skills.
* Take responsibility for promoting the health, safety, and security of patients, public, colleagues, and self.
* Respond to individual needs to maintain a safe environment and report any identified risks.
* The post holder will work according to Trust standards, policies, and procedures. No budgetary control but awareness of effective use of resources.
* Attend regular supervision sessions with your line manager.
* Attend yearly Achievement Reviews with your Line Manager.

Qualifications and training
* NVQ level 2 with evidence of working towards level 3
* NVQ level 3 in Health
Knowledge and Experience
* Ability to maintain accurate written records
* The ability to liaise efficiently and effectively with patients/carers, a wide range of health and social care workers, GPs, and formal and voluntary care providers within the primary care setting
* Broad range of simple nursing procedures e.g., Venepuncture, BP monitoring, basic wound care
* Experience of working within the primary care setting
Specific Skills
* Basic IT skills
* Excellent communication and interpersonal skills
Requirements due to work environment/conditions
* The ability to work independently in a potentially unpredictable and occasionally highly unpleasant working environment, to include body fluids and working in extreme weather conditions
* The ability to be flexible, according to patient and workload requirements
* Full driving licence/Access to Transport
