An exciting opportunity has arisen within the Kingsley Ward Team, our inpatient dementia assessment unit at Hollins Park Hospital, Warrington. Our ward provides a high standard of assessment and treatment for those with Dementia and BPSD (Behavioural and Psychological Symptoms of Dementia).
You will gain valuable experience and skills across the field of Dementia, incorporating both physical and mental health. You will be motivated to make a real difference in the lives of people living with dementia, where passion, empathy, strong communication and excellent teamwork are essential qualities for this role.
Whilst work experience is desirable, it is not essential as full induction and training will be provided alongside ongoing support from our experienced team.
This post is subject to an enhanced check with adults barred list Disclosure and Barring Service (DBS).
Main duties of the job
1. Responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care.
2. Act as Named Nurse as required.
3. Ensure that the nursing practices of the clinical area comply with the Trust’s policies and procedures.
4. Participate and assist in setting standards of service user care based on sound research findings.
5. Participate and assist in systematic monitoring and evaluation of nursing practice within clinical area.
6. Carry out nursing care based on the principles of individual care programmes/nursing process in accordance with specialist integrated Care Pathways.
7. Assist in the development and implementation of integrated care pathways, including appropriate service user assessments and models of care.
8. Support the implementation and maintenance of team working.
9. Participate in the process of potential new admissions to the service, including the assessment of such individuals in accordance with the agreed care pathway and completion of quality reports where appropriate.
10. Develop therapeutic relationships with service users and carers.
11. Participate in case supervision to monitor standards of service user care and documentation.

Detailed job description and main responsibilities
1. To coordinate the care of service users carrying out assessment planning, implementation and evaluation of their care under the guidelines of the care programme approach and under the supervision of the line manager.
2. To ensure that the service users care plan is developed to meet the assessed need of the individual and that unqualified staff are delivering care as laid down in the care plan.
3. To liaise with family members, carers and significant others when completing the assessment and developing a care plan.
4. To develop a risk management plan for service users, taking note of the contributions from other members of the team.
5. To direct unqualified staff in order to ensure delivery of a high standard of care to service users.
6. To participate in setting standards for care based on sound evidence based and research findings.
7. To participate in the systematic monitoring and evaluation of service users that you have responsibility for with guidance from a supervisor.
8. To communicate with other members of the multi-disciplinary team to ensure the communication of any relevant information which has an impact on the service user’s care.
9. To use verbal and non-verbal communication skills gained through training or experience to impart sensitive information.
10. To check understanding of information where there are often barriers to effective communications due to distress, psychiatric symptoms or where English is not the first language.
11. To use skills gained through training and experience to give and receive information to other individuals with regards to a service user's care in the reviews and multi-disciplinary meetings.
12. To have knowledge of the Mental Health Act / Mental Capacity Act and other relevant legislation.
13. To provide and coordinate the provision of a range of therapeutic activities for service users and monitor their effectiveness.
14. To take responsibility for the safe administration of medication to service users ensuring that the Trust policies and procedures and the Nursing & Midwifery Councils regulations are adhered to.
15. To ensure that all relevant information with regards to a service user's care and treatment is documented accurately and legibly into the health record.
16. To under the guidance of a supervisor provide reports both in written and verbal formats as requested.
17. To contribute to teaching sessions in order to pass on knowledge and skills.
18. To use skills gained through training and experience to de-escalate situations where service users become physically or verbally aggressive.
19. To ensure that unqualified staff members get appropriate support following violent or other untoward incidents.
20. To ensure that all incidents or near misses are documented as per Trust policy and that any follow up action is taken.
21. To actively participate in service user reviews ensuring that service user needs are met.
22. To develop care plans that enable service users to reach and maintain their optimum level of health and independence.
23. To react pro-actively in cases of service user crisis and put in place immediate plans to resolve the situation with supervision and guidance as appropriate.
24. To assess and monitor the physical and mental health of service users, seeking advice and support from medical staff as appropriate.
25. To actively participate in team meetings in order to positively influence team working and the development of research based practice.
