Job summary Due to expansion The Integrated Complex Assessment Team have a vacancy for a Complex Assessment Nurse. The integrated Complex Assessment team is responsible for the assessment and tracking of patients within a (D2A) discharge to assess bed based service within a timely manner relieving the pressures to the acute hospitals. The role involves working closely with multi-disciplinary teams, service users and relatives performing early review of service users transferred into assessment beds to establish needs and funding requirements for discharge. Following the national framework for continuing Healthcare and NHS funded Care the post holder will be responsible for the completion and sharing of timely assessments and funding applications The Post holder should be able to prioritise their own workload to ensure effectiveness and efficiency in delivering objectives whilst working with minimal supervision. Being proactive and able to demonstrate good organisational skills and excellent communication skills both in verbal and written correspondence. The role is varied and requires confidentiality, initiative, flexibility and capability of working under pressure and to deadlines. The Post holder must be able to travel over the D2A footprint and have excellent written and verbal communication skills.
