Job summary As a Case Manager, the post holder will work as part of the Adult Secure Provider Collaborative commissioning team in delivering an effective service to ensure safe and effective care pathways are in place for all service users within Kent, Surrey and Sussex in specialised mental health care settings within the NHS and Independent sector. Main duties of the job To provide clinical support to specialised commissioners regarding the defined population, this will include projected mapping of future service use, identification of users for future service Scope & Authority and planning for future service needs. The role supports the business in driving transformation and value for money in planning, commissioning, and service. It is designed to build a combination of subject matter expertise and technical skills to develop strong service delivery. This role will likely specialise in either Low or Medium security for men. Having a specialism will allow the Case Manager to develop a strong level of expertise in case management within the specific level of security and offer expertise knowledge in the future planning and development of the secure estate.
