Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. Trusted Homecare is a young, vibrant Company based on the Isle of Wight who began supporting individuals in our community in February 2024. Our office and Caregiver team is friendly and supportive and our Care Coordinator will be warmly welcomed and supported through their new role. Experience as a senior carer be an advantage.
Working as a Care Coordinator demands excellent time management, good administration and IT skills plus a pleasant telephone manner. Experience of using electronic care management systems would also be an advantage.
Responsibilities
1. To work alongside the Registered Manager in ensuring the smooth and effective running of daily care operations.
2. To work with the Registered Manager in ensuring compliance with Regulations and CQC requirements.
3. To ensure the scheduling is carried out efficiently enabling the Caregivers to spend the allocated care time with each Client and to allow for travel time.
4. Develop and maintain client care plans, ensuring they are updated regularly and reflect the individual's evolving needs.
5. Conduct regular check-ins with clients and Caregivers to ensure the delivery of high-quality care and client satisfaction.
6. To maintain the integrity and accuracy of all data stored in the computer and filing system and ensure that all bookings are correctly recorded and information is kept up-to-date at all times.
7. To deliver care from time to time when emergencies, or requirements present such a need.
8. Participate in the on-call rota which is held between the Registered Managers, Directors and Care Coordinator.
9. Following up on any concerns raised by Caregivers and liaising/referring to appropriate professionals, including GP's, OT's and DN's.
10. Participate in team meetings to discuss patient care strategies and improvements.
Qualifications
1. Previous experience as a senior care assistant.
2. A full driving license and access to reliable transport is essential for this role due to working in rural locations and accessing clients at short notice.
3. Strong organisational skills with attention to detail.
4. Excellent communication skills, both verbal and written.
5. Ability to work collaboratively within a team environment.
6. Proficiency in using office software applications (e.g., word processing, spreadsheets).
7. A compassionate approach with a commitment to providing high-quality patient care.
8. Relevant qualifications or certifications in healthcare coordination or administration are advantageous.
