West London Forensic Service has been chosen by NHS England to pioneer a new approach to support the discharge of Service users in Learning Disability and Autism medium and low secure forensic mental health settings and support their transition into community care. In the past, many people with learning disabilities have been placed in hospital units far from their home, family, and friends. The government has stated this must end, and people should receive support and treatment close to home. This can improve an individual’s quality of life, reduce inequality, and give hope to those who may have been detained for many years. It also saves significant expenditure on hospital care, which can be reinvested into community services.
We are looking for a Band 7 CPN, who is an RNLD qualified nurse, to be part of this innovative team. Forensic experience is desirable but not essential for this role.
The post will be based in Ealing, St Bernard’s Hospital Site.
The post holder will lead and monitor the performance and care delivery of the nursing staff in the LDA Community Forensic Team, ensuring local professional leadership. They will participate in the review of clinical policies and guidelines relevant to the area and support and implement changes to practice. Additionally, they will liaise with other organisations and professionals to promote communication and good practice; they should be able to undertake audit activity and supervise other staff in the service area/care group. An understanding of psychosocial interventions, including psycho-education, wellness recovery action plans, and discharge planning, is necessary. The post holder will also be responsible for the assessment of care needs and the development, implementation, and evaluation of programmes of care for a defined caseload, maintaining associated records.
Furthermore, the post holder will support the team manager in providing effective leadership within a clinical team, ensuring a high-quality service within the resources available.
