Job summary Job Overview: We are looking for a Practice Nurse to join our team, working alongside our skilled doctors, healthcare assistant, and administrative staff. Main duties of the job Key Responsibilities: Chronic Disease Management: Support the management of long-term conditions such as asthma, diabetes, hypertension, and COPD, including assessments, monitoring, and patient education. Health Assessments: Conduct health checks, screening, and preventative care for all age groups. Immunisations & Vaccinations: Administer childhood, adult, and travel vaccinations and participate in seasonal flu clinics. Health Promotion & Education: Provide advice on lifestyle changes, smoking cessation, weight management, and sexual health. Phlebotomy: Take blood samples for testing as required. Patient Records: Accurately maintain patient records in accordance with practice protocols and confidentiality guidelines. Collaboration: Work closely with GPs and other members of the practice team to ensure coordinated, holistic care for patients.
