Job Description
Qualified Social Worker, Enfield Council, Adults Hospital Discharge Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team.
The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager. The responsibilities will include:
1. Take referrals for all patients currently in admission awaiting discharge, regardless of which borough they are resident in.
2. Integrated service with staff from Enfield Social Work, Occupational Therapists, Enfield CCG, Whittington Health, and Haringey Social Work Team.
3. Offering our service users a holistic approach to support and assessing all Enfield residents on hospital admission locally or out of borough providing whatever support is necessary to facilitate a safe discharge including CCG and Safeguarding processes.
4. Service delivers support to residents of Enfield who have long-term progressive health conditions, adults with physical disabilities, older people, and their carers. We work in a supportive and friendly environment in an appreciative way.
5. Liaise with other professionals.
6. 36 hours per week (flexible by negotiation). The IDT working hours are 8am – 8pm on a rotational basis across 7 days to deliver continuity of service provision. Some staff will be on site and some will be working remotely over the weekends.
7. 2 Month Temporary Contract.
8. Start date subject to DBS and reference clearance.

The successful candidate will need to hold a DipSW or equivalent, must be registered with the HCPC, have a current DBS, and be a car driver.
