We are looking for motivated and enthusiastic Registered Mental Health Nurses, Occupational Therapists, and Social Workers to work in the Working Age Community Mental Health Service, supporting service users' recovery within the community. You will be recovery-focused, have good risk assessment skills, and possess a good range of therapeutic interventions supported by evidence-based practice.
Our Community Mental Health Teams operate Monday to Friday, with core hours being 9am to 5pm, offering flexibility through a hybrid model of working and flexible working arrangements. We require motivated individuals with good initiative and organizational skills. You will work as part of a multi-disciplinary team providing interventions for working-age adults, ensuring that the service users' recovery goals are identified and central to their formulation and care planning needs.

You will have the unique opportunity to work alongside experts by experience, third sector partners, primary care, and adult social care to develop and improve services that tackle health inequalities and meet the needs of the communities we serve.
With access to appropriate clinical & management supervision, the post holder will develop and demonstrate a range of appropriate skills, including assessment (including risk assessment), formulation, safety planning, and clinical interventions. The role provides the opportunity to coordinate the care of service users throughout their engagement with the service and wider services if required. To achieve this, the post holder will acquire all the necessary skills to carry out the role, including practicing care co-ordination skills through shadowing opportunities & supervision.
You will work as a Care Coordinator and be an integral part of the Community Mental Health Team, undertaking shared roles & responsibilities such as initial assessments, care co-ordination, and team duty. In addition, you will be required to carry out specific assessments & interventions, develop and deliver complex service user-focused Care Plans, and provide excellent packages of care, supported by a Multi-Disciplinary Team & Formulation process. You will also be expected to employ leadership within your role and provide Clinical Supervision, Preceptorship, and support to junior colleagues. Student education & support is inherent to this role.

The post holder will acquire all the skills necessary to progress into the role of care coordinator, including practicing care co-ordination skills in the clinical setting, engaging in management and clinical supervision with senior clinicians, establishing priorities in workload in partnership with senior team members, and maintaining a commitment to person-centred care.
To support and carry out comprehensive holistic mental health assessments, formulation, safety plans, the planning, and implementation of appropriate care and outcome measures, and act as a positive role model, providing support to other team members.
