The Individual Commissioning Team is responsible for the assessing, commissioning and case management for individuals with complex health needs.
As a Duty Clinician you will provide the initial point of contact for the individual commissioning team, you will meet their families and be required to produce a thorough and accurate review of their Fast-Track pathway (face to face) which may then require completion of a Decision Support Tool. You will have to be able to make a holistic positive difference to the lives and experiences of those you assess. You will need to be fully competent in the National Framework for CHC including Fast-Track and FNC and operate within local and national requirements.
The role will support the Service Manager with ensuring that statutory responsibilities are met. This may include support with Duty Triage process if required.

You must possess a current NMC (Adult, Mental Health or LD posts available), England professional registration and have the ability to travel within the locality.
You will be a competent professional with clinical experience within your field of practice and have a full understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care, Who Pays guidance and relevant sections of the Mental Health Act and commissioning and contracting principles.
Ideally, you will be an experienced CHC assessor however, if you are an experienced Nurse, England registered professional and do not have CHC experience we would still like to hear from you.
• · The Duty Clinician will co-ordinate all hospital discharge referrals, CHC fast track and all urgent matters where the allocated case manager is unavailable. They will work as a daily point of contact for all matters requiring an urgent response.
