Job summary An exciting opportunity has arisen for a band 6 nurse within our Community Heart Failure Team working within Sefton Place Community Services Division providing care and treatment to patients across both North and South Sefton. This team is a division of Mersey Care NHS FT- an organisation driven towards integrated health care delivery with a focus on excellent patient care, delivered in the right place at the right time, with a workforce of highly skilled clinical staff. Mersey Care NHS FT is striving for a bio-social model of health care, with transformation and integration at the forefront. Ultimately the aim is to ensure that we improve services for patients and staff in an environment that learns from experiences. Current working hours of the team are Monday to Friday 9am - 5pm, however, there are plans in place for the service to shortly move to 7 day working, 8am to 8pm daily which will allow for greater flexibility of working for both the service and for the successful applicant. Main duties of the job The role is based upon providing excellent Heart Failure management to patients in their own homes and in our clinic settings. The team works to prevent unnecessary hospital admission, initiate and optimise anti heart failure therapies, up skill primary care colleagues and improve patient experience, as well as ensuring preferred place of care at end of life. The service is currently going through a period of exciting transformation and is working towards extending its service to incorporate an urgent care response pathway and will be working collaboratively with other specialist teams who are part of the urgent care response model (Respiratory and ICRAS) The successful post holder should be educated to degree level, have Cardiology experience, and excellent clinical and communication skills. The applicant will be expected to demonstrate an understanding of Heart Failure treatment, and symptom recognition. The applicant must be able to transport themselves around the community according to the business need and is expected to independently carry out home visits and clinics. The successful applicant will be supported throughout a competency framework, with opportunities to develop knowledge and skills to support further career progression.
