We are seeking a dedicated Prevention & Wellbeing Social Worker to join our team, focusing on early intervention and preventative services. This role is integral in providing support, supervision, and care guidance to individuals and families, helping them maintain independence and improve their overall well-being.
Key Responsibilities
1. Conduct assessments and develop support plans to prevent crises and promote well-being.
2. Provide supervision and care guidance, ensuring high-quality and person-centred interventions.
3. Work collaboratively with multi-disciplinary teams, external agencies, and community services.
4. Offer support to colleagues in delivering high-quality social work practice.
5. Deliver targeted interventions to individuals, families, and communities to promote resilience and independence.
6. Ensure all casework is compliant with relevant legislation and best practices.
7. Maintain accurate records and contribute to service development.
About You
1. Qualified Social Worker with Social Work England registration.
2. Experience in prevention, early intervention, or wellbeing services.
3. Strong assessment, planning, and interpersonal skills.
4. Ability to work collaboratively and offer support to the wider team.
5. Knowledge of relevant legislation and safeguarding practices.
