As aBand 7 AHP Urgent Community Response Clinical Lead (occupational therapist/physiotherapist, you will play a crucial role in assessing, coordinating, and delivering high-quality, patient-centred care to individuals experiencing acute episodes of illness. You will lead a multidisciplinary team to respond swiftly to urgent care needs, providing short-term interventions, and working closely with GPs, hospitals, and other community services.
Key Responsibilities:
• Lead and support a team of healthcare professionals to deliver timely and effective urgent care services.
• Undertake advanced clinical assessments to identify and manage patients’ urgent health needs.
• Develop care plans to support patients’ recovery and prevent unnecessary hospital admissions.
• Work collaboratively with other services and agencies to ensure seamless, integrated care.
• Provide clinical leadership, mentoring, and support to junior staff and colleagues.
• Ensure the highest standards of patient safety, dignity, and respect at all times.
