The post holder is a Clinical Pharmacist (Maternity Cover) who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Senior Clinician and Line Manager who will develop, manage and mentor them.
You will be required to work from The Cedars Surgery as well as one other Maidenhead GP surgery.
Fixed term cover will be 37.5 hrs per week.
Salary between £45,000 and £51,500 per annum WTE depending on experience and completion status of courses CPPE and/or IP.
Benefits include NHS Pensions scheme and 25 days AL and additional bank holidays per annum.
Main duties of the job
* Clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
* Completing the Clinical Pharmacist in Primary Care Pathway and become a prescriber to support the Practice Team.
* Be responsible for the pro-active care management of patients with complex polypharmacy.
* Provide specialist expertise in the use of medicines whilst helping to address public health and social care needs of patients and tackle health inequalities.
* Provide leadership on person-centred medicines optimisation and quality improvement.
* The role will have a patient-facing element either face to face or telephone - including chronic disease management, repeat prescription management and structured medication reviews.

Job responsibilities
* See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma).
* Undertake structured medication reviews with patients and produce recommendations for agreement on prescribing changes and monitoring.
* Work with care home staff to improve safety of medicines ordering and administration.
* Provide patient-facing clinics for those with questions, queries and concerns about their medicines in practices.
* Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
* Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need.
* Signposting to other services and/or healthcare professionals where appropriate, while working within scope of practice and limits of competency.
* Manage repeat prescription requests, resolving queries where possible within scope of practice.
* Ensure patients have appropriate monitoring tests in place when required.
* Maintain full and complete records of all patient contacts using appropriate clinical templates and coding.
* Provide independent information to patients regarding medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate.
* The post holder will be supported to develop their role to become a non-medical prescriber.
* Maintain own clinical and professional competence and be responsible for own continuous professional development.
* Work with practices to ensure full compliance with Care Quality Commission standards for safe and effective care.

Person Specification
Experience
* Minimum of 2 years post qualification experience.
* Experience of working in primary care (desirable).
Qualifications
* Mandatory registration with General Pharmaceutical Council.
* Masters degree in pharmacy (MPharm) (or equivalent).
* Membership of the Royal Pharmaceutical Society and/or working towards Faculty membership.
* Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
* Independent prescriber or working towards/intent of gaining independent prescribing qualification.
* Completed or studying towards CPPE and/or IP.
