Birchwood Medical Practice have an exciting opportunity for a practice nurse to join our existing, expanding and dedicated nursing team consisting of nurses, nurse associate, and healthcare assistants. This position is offered on a permanent basis, full or part-time. The successful applicant will work as part of a multidisciplinary team, accountable for the delivery and standards of care provided to patients ensuring safe, effective, person-centred care. The role will support the practice team in the delivery of clinical policies and procedures including that of infection control for the practice supported by the wider IPC team. Responsibilities will include; Undertaking health assessments, administering medication and treatments. Carrying out new patient health checks/medicals. Managing and treating long-term conditions, including, c hronic disease management of our asthma, hypertension, COPD and diabetic patients. Performing ECG's basic observations and venepuncture. Chaperoning duties. Infection control. Wound management. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Recording accurate clinical notes. Requesting pathology tests and processing the results, advising patients accordingly. We welcome newly qualified nurse applicants who would be willing to undertake training in long term disease management. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree and c urrent NMC registration with an active PIN. An experienced nurse or someone who is looking to develop themselves - e xperience in long-term disease management and/or wound care is desirable. Primary care clinical experience is preferred. Understand the importance of evidence-based practice. Knowledge of clinical governance. Understanding of infection prevention and control measures.
