Experienced Band 7 Clinical Specialist required to join our friendly and supportive Healthcare for Older People team (Nurse or Therapist)
We are looking for a highly experienced Clinical Specialist to work in our Health Care for Older People Team. You will be working in our frailty SDEC, ED, MAU triage and MAU, as well as outreaching into the community to help manage patients in our Acute Hospital at Home frailty beds. You will require enhanced assessment and holistic management skills and will be working closely with our Pathfinder and Acute Hospital at Home Frailty Team.

You will need advanced assessment skills and be able to work with a holistic mindset and across traditional professional boundaries. You will have the opportunity to support frailty education to colleagues and other teams and help drive service development to improve the care and management of older people with frailty.
You will work closely with both acute and community colleagues providing a link between acute and community care in order to provide seamless transition between hospital and home or other receiving destination. The role will be challenging and is time-sensitive in order to support patient flow through the acute hospital but it is also rewarding and varied. We require a forward-thinking, experienced, motivated, and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will hold.
