We also provide a full range of services at sites where people live. These services aim to provide care and treatment to individuals and their families and help people to maintain their independence and continue with their day-to-day lives as far as possible. We provide a range of in-patient and residential services for individuals who cannot be appropriately helped in a community-based setting. Within our learning disability services we work closely with a large number of supported living settings/residential care homes in partnership with housing associations. Many of the people we help are seen in their own homes by members of staff, and some people attend our clinics to see nurses, social workers, therapists or doctors. We provide treatment, care and help on an individual or group basis where support and guidance is provided. We also work alongside GPs and other staff in local health centres, or with staff from other organisations often in the voluntary sector. We often see people for short periods of time, providing advice and treatment which helps resolve the person’s problems. For people with more serious longer term difficulties we will support them and work with them for a number of years. As a Foundation Trust we work in partnership with Sheffield City Council and have formal agreements with them, called a section 75 agreement, to provide a range of social care services on the Council’s behalf. Through these arrangements, we have made good progress in developing ‘integrated’ services for the people of Sheffield - an important goal that is shared by ourselves and the City Council. We have placed great importance on working with other organisations to deliver integrated health and social care services to local people. In doing this we have aimed to reflect and provide for the diverse needs of the people and communities of Sheffield.

Job overview
The Perinatal Mental Health Service is a growing specialist service for women and their families in Sheffield. We are excited to have received investment to expand the service, and are now recruiting more staff to come and join our team.
You will need to be an experienced Mental Health Practitioner (we are inviting applications from a range of professional backgrounds) with a real passion for perinatal mental health. Experience in this field is not essential and full induction is offered, but you will be keen to develop a career in this area. You should have attained a level of experience proportionate to the competencies detailed in the job description, possess the skills as detailed in the person specification and be able to work autonomously and with partner agencies. We are a trauma informed service that seeks to understand presenting difficulties in the context of people’s whole lives and work with that to promote understanding. You will be a compassionate practitioner who wants to make a difference to families.

Detailed job description and main responsibilities
* To work with the other staff in the service to triage, assess and care plan for women presenting with perinatal mental health problems
* To include women, their families and carers in decision making and care planning
* To signpost or refer patients to appropriate statutory and non-statutory services
* To work closely with the external stakeholders to promote a good care experience for perinatal women
* To conduct routine and emergency biopsychosocial, risk and mental health assessments in accordance with service policy
* To safely discharge patients from the perinatal mental health service in collaboration with other perinatal mental health service staff
* To negotiate clinical input from other disciplines from within and external to the perinatal mental health service to ensure the provision of a clinically appropriate and multidisciplinary approach
* To develop close working links with other mental health teams and agencies to enable swift and appropriate transfer between services
* To contribute to the development of robust MDT care plans in consultation with patients, families/carers, community teams, primary care, children’s services and other appropriate service providers
* To work in line with Safeguarding Children and Adult policies and procedures, including participation in safeguarding meetings when required
* To participate in clinical supervision both on an individual basis and team/peer supervision (e.g. Reflective Practice)
* To be a source of specialised perinatal mental health knowledge for any person requesting this, e.g. midwife, health visitor, support worker, carer
* To assist in the writing and delivery of training to other NHS colleagues, voluntary organisations and other groups as required
