We have this exciting opportunity for a Band 7 Respiratory Nurse Specialist based in the community in Pembrokeshire.
This post is advertised as a Band 7, however applicants who do not currently hold all the necessary competencies to a Band 7 level are encouraged to apply and, if successful, will be appointed under Annex 21 rules whereby 75% of the top of Band 7 will be payable during the 12 months. Applicants would be supported to develop their competencies required to fulfil the role within 12 months. Following successful completion and successful attainment of skills and competences for the role, the post holder will then be paid at band 7 (minimum point on the scale). Failure to achieve the competencies in the designated timeframe could result in re-deployment.
As part of the Annex 21 process, there is no financial penalty to those who apply for this role where the percentage will cause a reduction in pay, subsequently a local protection would be put in place until the pay has caught up.
The post holder will contribute to the development and implementation of respiratory procedures within their sphere of responsibility. They will work as part of the multi professional team working across the primary, community and secondary care interface.
The post holder will be seen as an expert in their field holding additional specialist knowledge and experience and provide leadership in clinical practice for teams and peers.
Main duties of the job
The duties of this role will include:
1. Leading and promoting integration of respiratory care between primary, community and secondary care.
2. Managing a caseload of patients with respiratory needs using evidence based/client centred principles to assess, plan, implement and evaluate interventions.
3. Undertaking detailed assessment of patients with respiratory needs, interpretation of appropriate investigations and the formation of a plan of care in line with NICE/BTS and local guidelines.
4. Issuing individualised self-management plans for people with respiratory conditions and work with patients to make relevant changes to support the plans.
5. Providing appropriate follow-up and referral to other members of the Multi-disciplinary Team.
6. Performing physical examination, perform investigations, such as arterial and/or capillary blood gases, peak flow measurements, spirometry and airway reversibility studies and after training interpret results and act on findings.
7. Completing relevant documentation e.g. HOOF to ensure safe administration of home oxygen.
8. Working in partnership with other members of the Multi-disciplinary Team.

* Registered Nurse Part 1 NMC register
* Holder of a degree or equivalent qualification or relevant experience
* Recognised advanced level respiratory course to Masters level or equivalent
* Expert knowledge in the field of respiratory care and demonstrable practice, including COPD, ILD, Bronchiectasis and home oxygen therapy
* Sound knowledge of the legislative and policy framework underpinning the delivery of an integrated community model e.g. The All Wales Community Nursing Strategy, appropriate National Service Frameworks and NICE Guidelines
