Key Responsibilities
Blended Role: Work across our domiciliary care service and outcome-based services, providing support to service users to achieve their goals and live independently.
Field & Office Duties: Conduct reviews, spot checks, initial assessments, and drive service improvement to maintain the highest standards of care.
Caseload Management: Oversee a caseload of service users and team members, ensuring exceptional care and support are consistently delivered.
Supervision & Mentorship: Carry out supervisions, support staff development, and provide direct guidance to team members to foster a culture of growth and excellence.
On-Call Participation: Join the on-call rota to provide timely assistance and problem-solving support outside of regular hours.
Collaboration: Liaise with your manager, social services, occupational therapists, and other professionals to ensure holistic care.
Service Support: Assist the coordinator as required and cover care calls when necessary to maintain seamless service delivery.
Community Engagement: Represent the company by participating in local community events and fostering positive relationships.
Maintain Standards: Uphold company values and adhere to the code of conduct, ensuring outstanding care is at the heart of every action.
What We’re Looking For
Driving Licence: A full UK driving licence is essential for this role.
Team Player: Strong communication and organizational skills to effectively support service users, staff, and fellow professionals.
Experience: A background in community care, team supervision, and service improvement within a similar setting.
