Job summary
An opportunity has arisen to recruit a band 7 Mental Health Nurse Practitioner for a 12 month FTC to cover maternity leave within the Memory Assessment Pathway, based in Stafford.
Applicants should possess the knowledge and skills to enable them to work in the memory and dementia speciality, working with service users affected by all types of dementia, including those with challenging and complex needs associated with a dementia diagnosis. Applicants must have a non-medical prescribing qualification.
Applicants will be required to work closely with the existing multi-disciplinary team members, providing diagnostic assessments and follow-ups to those referred. The role will include close collaboration with physical and social care colleagues along with our GP and primary care colleagues.
Main duties of the job
1. Delivery of a high standard of nursing care to individuals referred to the Memory & Dementia Service.
2. Provide a specialist mental health nursing assessment, care planning, and evaluation for those experiencing dementia.
3. Work in partnership with fellow team members and internal/external partners.
4. Undertake personal professional development and ensure compliance with mandatory training.
