Community Nurse - Truro Community Nursing Team

We have an exciting opportunity for an experienced, caring and motivated Community Nurse to join the Truro Community Nursing Team.
We are seeking a dynamic, forward-thinking professional who wants to help us deliver exceptional community care. A commitment to team working and providing excellence in patient care is a must as are exceptional communication and people skills.
You will need to be able to demonstrate real enthusiasm for community nursing as well as a desire for ongoing professional development.
Car owner/driver and the ability to use a car for work are essential.
Main duties of the job
1. To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures and guidelines.
2. To support all locality teams by working in a flexible way. Moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.
3. Responsible for the assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.
4. To work in partnership with patients and carers to offer support and advice and promote informed choice. Support the role of the Community Nurse Team leader.

Person Specification
Education, Qualifications and relevant experience
* Registered Nurse - Adult
* Current Professional Registration - NMC
* Evidence of recent relevant personal professional development
* To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
Other
* A valid UK driving licence is required for this post and access to a vehicle
