Purbeck Health Centre in Milton Keynes has a fantastic opportunity for an experienced practice nurse to join their well established team. This position is offered on a full or part-time basis with hours between 30-37.5 per week. The ideal candidate will provide high-quality patient care and support in a medical practice setting, contributing to the delivery of quality healthcare services. The main responsibilities of the role include: Undertaking health assessments, administering medication and treatments, and performing diagnostic tests. Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad. Running asthma, COPD and diabetes clinics and reviews. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree. Current NMC registration with an active PIN. Competence to undertake asthma, COPD reviews. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.
