Home First Community Support is a very busy team that is fast-paced and has lots of different functions as noted previously, although our main function is Rehabilitation and Reablement we do support the crisis response, hospital discharges, falls and assistive technology.
The Duty Office is manned 24/7 and is the hub of all operations and source of support for people.
Non-contact time is spent at Billet Lane, our registered base. We use this time to make sure we are always ready to respond to crisis in the community, update training, submit referrals, complete case notes, meet with your supervisor and teams.
A typical day in the life of the Reablement Assistant
* Shifts are 7am to 1.35pm and 4pm to 10.30pm and involves you following a set rota travelling to people’s own homes, delivering their care and support. You would typically support between 5 and 7 people per shift with their routines of daily living and maximising independence.
* The nature of the support includes personal care, support with mobility, medication, nutrition, therapy, community inclusion, working with and referring for aids, adaptations and assistive technologies.
* You will complete, follow and update support plans, complete case notes, audits, review and risk assess various forms as part of your daily routine.
* You will be issued with your own works smart phone and laptop to support your day-to-day work, completion / scheduling of allocated tasks, sending and responding to messages / emails.
* You will be accessible and responsive – ready to respond to Crisis e.g. with the lifting cushions, pendent alarm calls, community referrals.
* You will support families and individuals as part of their support with accessing information and signposting to other agencies.
* Entry level posts consist of reablement assistant posts both days and nights (these posts are within our transition and roving night services).
* Day posts are 25 hours per week and nights are 24/16 hours following a rolling rota.
