Who You Are You are a qualified social worker with a strong understanding of outcome-focused social work practices. You have in-depth knowledge of the Social Services and Wellbeing (Wales) Act 2014, and the Mental Capacity Act 2005, along with their practical applications. Your excellent assessment skills enable you to effectively engage with service users, focusing on promoting the wellbeing of older adults and their carers. You have experience in establishing and maintaining effective multi-agency collaborations. What the Job Involves You will be a part of the Older Persons Team South, supporting individuals over 65 and their carers in the Hay and Talgarth, Brecon, Crickhowell, and Ystradgynlais localities. Your responsibilities will include conducting assessments and reviews, helping individuals find appropriate solutions during personal challenges and for long-term planning. Safeguarding individuals from abuse and neglect is essential. You'll collaborate with various agencies, the third sector, and health colleagues to provide timely interventions.
