Job summary A very exciting opportunity has arisen for an experienced administrator/admissions coordinator to join the specialist Paediatric Surgical Team as a Patient Pathway Coordinator (PPC-Admissions). The role is currently offered as a full time substantive role and is based at the Denmark Hill site. The successful candidate will primarily be supporting the Paediatric Surgical and Paediatric Respiratory Team(s) who look after patients cared for by the Child Health department. They will also work as part of a wider administrative team to support the multidisciplinary clinical and administrative teams to provide a high-quality administration service, a positive patient experience and the highest levels of customer service. The post includes liaising with multidisciplinary teams regarding pathway coordination - including the coordination and management of referrals, appointments, and relevant documentation on the electronic patient system (EPIC). In addition to pathway management and administration, they will also be required to act as first point of contact for patients and other service users and will proactively deal with telephone enquiries from patients and colleagues in a professional manner. In this post, you will be required to cross cover specialities during busier periods or absence of staff. Main duties of the job Coordinating Patient Care To monitor and review patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies. To ensure that pathways are correct and actively managed and any procedures are booked and appointments are made in line with the Trust's Access Policy. To proactively find resolutions to improve the speed of treatment by working directly with clinicians and managers and escalate any breaches as required. Communication and Patient Liaison Communicate clearly, effectively and compassionately with the multidisciplinary team, patients and their family, visitors or carers, in line with the Trust's values. Supporting Clinical Team(s) Carry out designated administrative tasks under direction and with guidance, and following agreed standard operating procedures to support the delivery of a high quality, safe and compassionate healthcare service.
